Case Facts: On 9/6/2001, Melisa had a tubal cautery with burns over a 3 cm length of her tubes. On 2/15/2010 Dr. Richard Levin performed a bilateral microsurgical tubal ligation reversal (tubal reversal) with tube lengths of 8.0 cm on the right and 7.0 cm on the left.

Hello, my name is Adam Phillips. I am a soldier in the U.S. Army and I'm writing you to thank you for the successful surgery you performed on my wife Melisa Phillips on Feb 15, 2010.

I was deployed to Iraq while she had the surgery and we wanted to conceive as soon as I got back to the states so that I would have as much time as possible with the new baby before my next deployment. You can imagine how excited we were to find out Melisa was pregnant a few weeks after I returned and that the doctor thought the date she conceived was July 15, 2010, the same day I returned from Iraq.

My wife had three children from a previous marriage and I had none of my own so it was important to us that we got to share the experience as much as possible. We now have a beautiful baby girl named Evelyn Rose Phillips (Evee for short) born April 15 2011, she was 9 lbs 11 oz. 22.5 inches long. She is three months old and it looks like I will get to be here for her first birthday.
