Steps required to print out your prepayment agreement:
- In the left most column labeled "height" find your correct height.
- Slide straight across the row containing your height until you find the weight range that includes your current weight.
- Click on your correct weight range for your height and your prepayment agreement will appear on the screen.
- Both you and your significant other must sign the document.
- Add a check made our to Richard M. Levin, MD., PSC. for the amount on the agreement.
- Mail the agreement and the check to:

- Repeat the above 2 steps until the complete payment has been made. You can make larger payments or more frequent payments if you want to have the surgery sooner than 24 months. Once the full payment is made, call 1-800-766-0304 to set up your surgery date.
