Tubal Reversal - Fast-track:   |

We are now offering a Fast-Track tubal reversal option for those of you who want to save both time and money getting on the surgery schedule. The elements of this approach are somewhat different than our regular program so I will give you an exact outline of how you should proceed. Potentially you could accomplish everything that needs to be done in about one hour! If you need questions answered with this program please email Melanie at melanie@babies-by-levin.com or Linda at linda@babies-by-levin.com as they are the people who receive your sterilization records, medical histories and other documents before Dr. Levin. It's quicker for them to ask Dr. Levin about any issues you might have.
- Must do (First): If you have not already done so then fill in the on-line initial contact form so we can be in touch with you.
- Must do (Second): Fill in our personal data and medical history form. Save the form to your "desktop". After it downloads then click on the item on your "desktop" to open it. When you have filled in all four pages, re-save it to your desktop, attach it to an email and send it to me at history@babies-by-levin.com.
- Must do (Third): Download the medical records release form to your desktop. Fill in all of the blanks and then print it. Sign and date the release and then mail it to the hospital/clinic where you had your sterilization performed.
- Should do: There are a couple of lab issues that you may wish to consider. Women 40 years of age or over should have their blood drawn on cycle day 3 for FSH (follicle stimulating hormone) to check for "ovarian reserve". This term relates to the ability of your ovaries to make and pass eggs that are capable of conceiving. You can contact Melanie or Linda (below) for an order to do the FSH test and you should make sure to know that the result is normal before scheduling your reversal.
The second test is a sperm count to make sure your significant other is currently fertile. This can be done by having a sperm count done at a lab near you. Please contact Melanie at melanie@babies-by-levin.com or Linda at linda@babies-by-levin.com to arrange either of these tests.
- Must do (Fourth): Check with Melanie at melanie@babies-by-levin.com or Linda at linda@babies-by-levin.com to make sure we have received your medical records and your medical history form and that Dr. Levin thinks you are a candidate for the tubal reversal procedure.
- Should do: Please read the 11 page section about tubal ligation reversal so that you understand what it's all about, what it costs and what your chances of success could be. By reading all of this you will avoid having to wait on a telephone consultation with Dr. Levin and move more quickly on to the actual reversal procedure.
- Should do: Watch this pictorial walk through of what your surgery day will look like!

- Should do (Fifth): Join our next Reversal Telephone Conference to hear Dr. Levin talk about the tubal reversal process by calling 1-800-391-1709. Listen to the greeting and then enter the "bridge number" 249984 from your phone to enter the conference call with Dr. Levin and other women who are getting ready to have a tubal reversal. You must sign up for this meeting by e-mailing Melanie at melanie@babies-by-levin.com or Linda at linda@babies-by-levin.com and giving them your full name and the phone number from which you will be calling.

Each conference will start at 12:00 Noon (Eastern Time) / 11:00 AM (Central Time) / 10:00 AM (Rocky Mountain Time) or 9:00 AM (Pacific Time). You can enter the conference at 2-3 minutes prior to the hour but the door will close and lock at 5 minutes after the hour when Dr. Levin will start so don't be late. Your voice will be on "mute" at various times so that Dr. Levin can talk but you can "raise your hand" to ask a question by pressing "5*" on your telephone. Dr. Levin will get to you at some point during the conference to give you an answer that everyone can hear (so please make your question of general interest, not just specific to you. Dr. Levin makes individual personal calls right after the meeting to some of the women who have issues that he has identified. If you have something you wish to discuss personally this would be a good time to do that. Just make sure to tell Dr. Levin during the conference or let his staff know ahead of time that you require some personal time. Dr. Levin and his staff are always available to speak with you should you have the need!

- Must do (Seventh): Once you know we have the payment call 1-800-766-0304 to lock in your reversal date.
- Must do (Eighth): Bring the other ½ of your tubal reversal fee with you to the surgery as CASH - in a name-labeled envelope and give it to Dr. Levin or his assistant Venica when you meet each other in "pre-op" holding. If you do not have the balance of the fee (in cash) then the procedure will be cancelled and will not be rescheduled.
